To be eligible for benefits, members whose health center has an ophthalmologist or optometrist must have vision exams provided by their health center vision provider. If a member's health center does not have a vision provider, routine vision exams may be obtained from optometrists and ophthalmologists in HMSA's HMO Vision Network. Coverage of dependent children of divorced or separated parents is determined according to special rules.
